New Patients

If you live within our practice area you can register with us by calling into either of our surgeries during opening times to collect the forms from our Reception Team, or alternatively click on the "Registration" Tab to download and print the Registration Forms. Please fill in all the boxes on the New Patient Forms and hand into Reception. If you need help in completing the forms please do not hesitate in speaking to one of our reception team who will be happy to help.

We like to meet all new patients so you are invited to arrange a new patient consultation within one months of registering. This is an opportunity for us to obtain important medical history about you prior to the arrival of your medical notes. 

  • If you are on regular medication you will be seen by our Health Care Assistant but also have an appointment with your doctor
  • If you are NOT on any regular medication you will be seen by our Health Care Assistant 


If you are on medication and have an appointment with the doctor please bring with you the printed list of your current medications from your last GP with any relevant letters.

Recorded Telephone Calls

Please note all of our calls in and out of the surgery are recorded. We may monitor, record, store and use any telephone call with you in order to check any instructions given to us, for training purposes, for crime prevention and to improve the quality of our services.

We want to make sure that you can read and understand any information that we send you, or if you need support in communicating with us.

Please complete the "Accessibility Information" section on our Registration Form or alternatively let a Receptionist or any other member of staff know.

GP2GP

When a patient newly registers at a GP surgery their medical records have to be transferred from their previous surgery. GP2GP is an approved facility which enables patient’s records to be electronically transferred directly and securely between surgeries.

Why is it needed?

GP2GP allows the transfer of medical records to be more accurate, secure and a lot faster than the current paper-based approach, which can take weeks to complete. This is helpful for GPs because it provides full and detailed medical records to be available for a first consultation (new patient appointment).

GP2GP will save a lot of time and paper and will generally help GPs and their teams to provide patients with a safer, more efficient service and continuity of care.
