Please remember, even if you register for any of the additional online services, you still have the option to contact the practice by phone - this is simply an additional service which many patients have found more convenient.
'Try the NHS App
If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet.
You can use the NHS App to check your symptoms and get instant advice, book appointments, order repeat prescriptions, view your GP medical record and more.
If you already use Patient Access or Evergreen life you can continue to use it. You can use the NHS App as well.

Available 24 hours a day
More information, including leaflets for getting started and easy-read guides can be found on the NHS Website.
If you wish to register, or if you are already registered on Patient Access but wish to view your medical record, then you will need to see one of our receptionists with the following identification:
Two forms of VALID photo identification, for example
- Photograph UK driving licence
- Student card
- Disabled badge
- NHS Card
If you only have 1 form of photo identification (listed above) then you will also need to bring:
- Bank statement
- Credit card statement
- Building Society statement
- Mortgage Statement
If you have no form of photo identification then we will need to see 2 of the above.
We are requesting this ID to protect the security, privacy and confidentiality of your medical record. We will then add the appropriate verification code to your medical record to enable this access.
Viewing your medical records online
