If you are over 75 and have been recently admitted and then discharged from hospital you may get a call from Stuart Coxon who is our Lead Community Nurse.

“There is no place like home” so his aim is to make sure that patients have the correct care in place to support them when at home. His work focuses on the more vulnerable members of our elderly population, including those already in residential or nursing homes, to co-ordinate and ultimately optimize the care they receive.
Stuart is available Monday – Thursday 8.30am – 3.00pm and should you need to contact him please leave a message with the Reception Team and he will get back to you as soon as possible but within 3 working days. If your call is urgent then it may be more appropriate for the receptionist to pass your information onto the duty doctor.
