When a baby is born and discharged from hospital care the parents should receive a discharge summary. The parents should bring the discharge summary showing the baby’s name, NHS number and ethnicity into the
surgery for the baby to be registered with a GP. The GMS1 Registration Form available in your Bounty Pack (or under the Registration tab) will also need to be filled in to complete the registration process.
