Each patient who is registering will need to complete a
- GMS1 Registration Form
- New Patient Questionnaire
- Summary Care Record
If you are registering children please make sure that you print and complete the Health Visitor Form, you will only need to print one of the health visitor forms as there are boxes available for all family names.
You may then present the forms to Reception at the surgery who can formalise your registration.

Please remember – you need to make a new patient appointment within one month of registering.

Patient Access
When you visit reception with your completed forms, you can also ask about Patient Access. This is an online system for booking appointments with a doctor and re-ordering your repeat medication. Photo ID is required to activate this service, please see ‘Patient Online Access’ found under the ‘Further Information’ tab on the right hand side of this page. Unfortunately this service is not available for patients who use our branch surgery at Newton Flotman, we are still waiting for this function to be added to our computer programme by our system supplier. Confidentiality is at the forefront of everyday life within the practice, so please remember we cannot share any of your details, whether it is appointment times or results with anybody else. If you wish to share your information with a member of your family, or close friend so they can ring on your behalf, please complete the form below and hand into reception.
